In an effort to keep your child, and all children and adults in the building, safe at Oak Mountain Elementary School, we are taking extra precautions this year. As per ADPH guidelines, students presenting with symptoms of COVID-19 must be isolated and sent home. These symptoms include fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea or vomiting, and diarrhea. PLEASE help us by screening your child at home each morning and do not send him or her to school with any of the above symptoms. Use Habit 1: Be Proactive- Please make sure we have your updated contact information so that we may reach you during the school day, make a Plan B (and C, D) for who will pick up your child from school if you will not be able, and keep your child home with any sign or symptom of illness. Registration of Returning Students
- RETURNING students will need to complete forms using the S.C.O.R.E. online forms program. You will need your child's personal access code provided by the school to complete the Returning Student forms. If you do not have an access code, please contact the school.
- Submit Proof of Residence - Scan or photograph and upload the required enrollment documents in S.C.O.R.E. The school will verified that your child is eligible to enroll and direct you to additional forms which may be completed online or on paper. These forms are school specific and, therefore, cannot be completed in advance. More Information
